More than 80 out of 100 people (more than 80%) with stage 2 bowel cancer (also called Dukes’ B) will survive their cancer for 5 years or more after they’re diagnosed.

Colon cancer in the second stage has spread to the outer wall of the colon or may extend to nearby tissues or organs, but in this stage it does not spread to the lymph nodes or distant organs from the body.

The second stage of colon cancer is divided into 3 other sub-stages as follows:

  • Stage 2A: At this stage, the cancer has spread to the outer lining of the colon.
  • Stage 2B: The cancer has grown beyond the outer lining of the colon into the lining of the abdomen that covers the organs in the abdomen and is known as the peritoneum.
  • Stage 2C: This stage means that the cancer has grown through the colon wall into nearby organs and tissues.

Symptoms of colon cancer stage II

Colon cancer often does not cause any symptoms in the early stages, they may not appear until after the disease has progressed to the second stage or beyond, and symptoms may generally include the following: 

  • Change in the frequency of bowel movements.
  • Constipation.
  • A change in the consistency of stool, so that it is loose or watery.
  • Blood in the stool, either in the form of bright red spots or dark, tar-like stools.
  • Rectal bleeding.
  • Pain or cramps in the abdomen and bloating .
  • A constant feeling of not being able to completely empty the intestines.
  • Tired.
  • Weight loss.

Diagnosis of colon cancer stage II

Some diagnostic tests play a role in staging colon cancer, including the following:

  • biopsy;
  • Magnetic resonance imaging.
  • Computerized tomography .
  • Ultrasound examination.
  • Chest X-ray.
  • Lymph node biopsy.
  • surgery.
  • Complete blood count.
  • Carcinoembryonic antigen test .

In fact, surgery is the main treatment for colon cancer stage II, but chemotherapy and radiotherapy can also be used in some cases, and these treatment options can be clarified as follows:

1. Surgery

Surgery usually involves removing the intestine along with the lymph nodes. The type of removal depends on the location of the cancer.

It may also be resorting to one of the following procedures after resection of the intestine, depending on the location of the cancer and the health of the remaining part of the colon to create a new path for stool to leave the body:

  • Colostomy: An opening is made from the colon to the outside of the body through the abdominal wall.
  • Ileostomy: An opening is made to the outside of the body from the ileum.

And both the colostomy or ileostomy can be temporary to allow the intestines to rest and heal after surgery. 

2. Chemotherapy

Your doctor may recommend adjuvant chemotherapy after surgery if there is a high risk of cancer recurring due to certain factors, such as:

  • Cancer is abnormal when examined closely in a laboratory, meaning it is high-grade.
  • Cancer growth to nearby blood vessels or lymph vessels.
  • Remove at least 12 lymph nodes.
  • Cancer cells are found in or near the margins of the removed tissue with the tumor.
  • Colon blockage due to cancer.
  • A hole in the colon wall caused by cancer.

The main options for chemotherapy drugs that may be given to treat stage II colon cancer may include the following:

  • 5-fluorouracil (5-FU) with leucovorin.
  • Oxaliplatin.
  • Capecitabine.

It is worth noting that the patient may receive chemotherapy with or without radiotherapy if it is difficult for him to undergo surgery due to his health condition.

3. Radiation therapy

Radiation therapy may be given after surgery to help prevent the cancer from returning in the same area if:

  • Cancerous tumor growth into nearby tissues or parts.
  • The inability to remove all of the cancer by resection of the intestine.

Radiation therapy, with or without chemotherapy, can also be given to people who are not healthy enough to undergo surgery.

What is the survival rate for stage 2 colon cancer?

Colon cancer staging is useful in predicting the likelihood of cancer recurring after surgical removal or chemotherapy. Survival rates for stage II colon cancer range from 55-80%.
